General Insurance Information:
Voicewize is a contracted provider with the following insurance companies:
* Blue Cross Blue Shield
* Harvard Pilgrim Healthcare
* Medicare Part B
* Tufts Health Plan
Every insurance company provides a wide variety of plans. While speech therapy is a covered benefit of most plans, the extent of this coverage, deductibles and exclusions vary greatly.
As providers, we are not always able to obtain complete information regarding the details of your specific insurance plan contract. While we are happy to assist you in the process of using your insurance coverage, it is important that you understand the details of your plan (for example deductibles and exclusions) and to verify your coverage with your insurance company if you are not sure.
Insurance Referrals:
Your insurance plan may require that an "insurance referral" be recorded prior to coming to your visit. In order to obtain a referral, you must call your primary care physician's office and provide their Referral Coordinator with the following information:
* Name of service: Speech Therapy
* Full, legal name of our practice: Entertaining Diversity, Inc d/b/a Voicewize
* Our National Provider Identifier (NPI): 1558424994 (this is very important)
* Our fax number (781) 329 - 2207
* Any other information they request to identify you
Please make certain your insurance referral is recorded at least 4 - 5 days prior to your visit.
What to Bring to Your Appointment:
* Your insurance card
* Any relevant medical reports, notes or images you have that may be relevant
